Rates:

I am able to bill insurance for visits only for people located within Washington state. For visits where insurance is not being billed, a day of service discount is applied.

Billed to insurance in Washington State:

  • Initial Visit (60 minutes): $200

  • Follow Up (60 minutes): $170

Patient Pay / Day of Service Discount:

  • Initial Visit (60 minutes): $170

  • Follow Up (60 minutes): $150

I am a preferred provider for the following plans:

  • Regence

  • First Choice

  • Premera

  • Lifewise

  • Aetna

  • Kaiser Access PPO Plan (otherwise out of network)

  • Cigna

  • United Healthcare

I always recommend calling your insurance company to confirm that your specific plan covers nutrition services. You can give them the billing codes 97802 (initial visit) and 97803 (follow-up visit), and let them know the conditions you want to address and ask what your co-pay or co-insurance will be. This is the best way to verify your benefits and any out of pocket costs, as I’m not able to contact insurance companies on your behalf. 

There is very limited coverage for nutrition services under Medicare and Medicare Advantage plans. Please carefully check your benefits to make sure you have nutrition coverage.

If you’re paying out of pocket, I also accept HSA and FSA.  If needed, I can provide a superbill that you can submit to your insurance for out of network benefits.
